Travel Request Form
All travel requests MUST be turn in 3 business days prior to travel. If someone is missing on the checklist, this indicates we do not have a MIC on file & they are not a recognized Club Sports member.
Click on your team name, fill out the required information, click submit.
*If we see conflict with your travel request, your Club Executive will notify you ASAP to make adjustments prior to your travel.
